Changing an editable parameter may change the recommend dose, so please remember to create any dose adjustments in the calculator after adjusting stuff in the editable info section. Compensating for obese patients and creatinine clearance:Many of the online calculators for vancomycin kinetics still use actual or ideal body weight for the calculation of patients' creatinine clearances, and avoid adjusted body weight all-together. This application defaults to using an adjusted body weight for creatinine clearance in patients whose actual body weight is greater than 125 percent of their ideal body weight. Of course, this can be edited at the latest step of the calculator ( if you would prefer not to use the adjusted bodyweight in the calculation of creatinine clearance). This calculator also allows a comparison of the Salazar kinetics to the normal kinetics used. If a patient's actual body weight is greater than 190 percent of their ideal body weight, the calculator will ask if you would like to view these parameters.
