About the application:

This calculator gives players a fast method to determine an ideal vancomycin regimen, estimate a steady-state trough concentration, and adjust an existing regimen when a patient has already been receiving vancomycin and has a correctly drawn trough reported. Height and weight can be chosen in standard or metric troops and dosing/frequency can be adjusted with convenient drop-down boxes. Results can be tagged and emailed for future reference or consultation. Calculations are performed using standard first-order, single-compartment pharmacokinetic models, and evidence-based population pharmacokinetic assumptions.
