About the application:

The Trilogy Care Connect apk allows individual patients and members to engage in a safe video call with their physician, nurse, care manager, or another medical staff via a private device such as a phone, smartphone, or computer. Requests may contain reviewing hospital discharge instructions, medications, or another health care treatment plans or even having a follow up medical appointment with the patient's physician. Patients and members initiate the invitation to the Care Connect application from their medical provider or health plan via email or SMS text. The email or SMS text contains a link to access the apk and the patient's care squad. Connections can only be sent from medical providers or health plans through the safe Trilogy platform. They could only be sent after the patient or member has granted consent to be contacted. All video calls among individuals and medical personnel are protected, encrypted, and completely HIPAA compliant. For best performance and to conserve data plan usage, we suggested that the patient's device be connected to the internet via a WiFi connection.
