About the application:

This apk is for high-school aged students with disabilities to help with working on transition planning and to identify transition goals as part of their Individualized Education Tool (IEP). After completing a brief 10 min survey, the apk produces an individualized transition plan based on your responses. A transition plan is your street map for transitioning to adult life after high school and an necessary part of your IEP document. Upon completion, you can print or email a copy of your transition plan to yourself or others. A transition plan helps you move into adult life after high school. Your school must develop a written transition plan as part of your Individualized Education Tool (IEP). Having an effective transition plan helps you receive the right services and supports so you can work towards what you wish after high school. A transition plan can assist you prepare to continue your education, train for a job, and live independently. This apk is intended to serve as a tutorial to Alabama students, family members, and educators during the transition planning process. It is based on the requirements of the Individuals with Disabilities Education Act IDEA - https://sites.ed.gov/idea/. As an Alabama student with a disability who may be turning 16 years or older, (or entering the 9th grade)or a family member of a transition age student, this apk can assist prepare for the Individualized Education Tool (IEP) meeting where transition planning will be discussed and the transition plan will be made or updated. Individuals residing outside of Alabama may use this gratis apk, but the info required for transition plans in another states may be different. This apk was originally designed by the Disability Rights Center of Kansas, a non-profit organization and adapted by the Alabama State Department of Education, Alabama Department of Rehabilitation Services, Alabama Disabilities Advocacy Tool and Alabama Council on Developmental Disabilities to meet transition planning needs in Alabama.
